Notes: The opportunity has arisen for the appointment of two Consultant Geriatrician posts in Medicine. This is a new post within the Care of the Elderly team and will include working within Hospital at Home (HAH) service being developed. The Consultant Physician post is intended to strengthen the role of Care of the Elderly Medicine in meeting the challenges of increasing numbers of frail elderly people presenting to secondary care services with acute and complex illness. The appointee will be expected to assist colleagues in providing early comprehensive geriatric assessment and rehabilitation to such patients. The appointee will also have the opportunity to contribute to our established Geriatrician led Acute Frailty service. HAH will provide acute care on a short-term basis, for patients who are acutely unwell and in need of hospital level care. The HAH team will work closely with community services including primary care, NIAS, nursing homes, specialist nursing teams and community AHP teams, to embed pre-hospital referral pathways and promote communication and safe transfers of care. The new Hospital at Home service will be introduced in a phased manner and this post will bring opportunities to shape and develop the model within the NHSCT. A waiting list may be compiled for similar permanent/temporary, full/part time posts which may arise within the next 6 months.
