Notes: The SEHSCT is committed to the implementation of a Hospital at Home (HAH) and Frailty Service for all residents. Currently the HAH service operates in North Down and Ards, with planned expansion to Lisburn and Downpatrick localities to provide equitable service for patients across the Trust. An appropriate skill-mix of multidisciplinary professionals are being recruited to all localities to support and progress this implementation. HAH is a consultant led multidisciplinary team which provides an alternative pathway to acute hospital admission. It provides acute care on a short term basis, for patients who are acutely unwell and in need of hospital level care. The HAH team work closely with community services including primary care, NIAS, nursing homes, specialist nursing teams and community AHP teams, to embed pre-hospital referral pathways and promote communication and safe transfers of care
