We are recruiting to 3 vacancies within the District Nursing Service in the southern Trust.The postholder will work In SouthDown Integrated Care Team (ICT) as part of a multi-disciplinary team and work very closely with social work, occupational therapy, physiotherapy colleagues. The postholder is responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care across a 7 day service covering from 0.900-22.00 (in the first instance with progression to 23.00). The staff will be required to work from Kilkeel primary care centre or Warrenpoint Health centre however may need to work from Rathfriland depending on service demands. In the main you will be in one area. Core team hours are 0.900-17.00 and during the induction process you will work within these hours. A waiting list may be compiled for future posts within South Down Community ONLY.
