Here is an exciting opportunity to work as an IMPACT facilitator to help shape future preventative work with older people within the Southern Trust. It is a part-time, one year post based within the adult community service team, Bannvale House, Gilford, Co. Armagh. Facilitator will draw on research evidence; and on the experience of older people, family carers and health and social care staff, to explore what types of supports or interventions help to prevent or reduce deteriorating health and social circumstances; and on this basis propose how current services might be adapted or new approaches introduced within the Southern Trust, and beyond, to keep older people at home for longer.
