Band 4
Community Access Support Worker
In this role, you will work directly with children and young people with disabilities in a variety of social/leisure and recreational activities with aim of promoting personal development, achievement, social inclusion and integration.
Social Care Support Worker
In this role, you will be responsible for the delivery of family support plans and family time, under the direction and supervision of a Social Worker Manager. You will provide timely, accessible support and help to children with disabilities and families based on assessed need.
Band 5
Outreach Worker
In this role, you will work as part of a wider multi-disciplinary group across the Children’s Disability Service to provide flexible, timely, responsive community based supports, interventions and early help to our service users, who are children and young people with a learning disability or complex health and/or behavioural needs and their families.
Band 7
Outreach Team Leader
In this role, you will manage and lead the CWD Outreach Team, and work as part of a wider multi-disciplinary group to lead and co-ordinate the delivery of flexible, timely, responsive community based supports, interventions and early help to our service users, who are children and young people with a learning disability or complex health and/or behavioural needs and their families.
