Notes: This post for a Consultant Geriatrician who will be based at the Downe Hospital. The Consultant Physician will be part of a team working in a district general medical unit. There is a focus on acute geriatric care and on chronic disease management. The Downe Hospital has been at the fore of implementing modern working practices. Secondary and primary care work together using innovative schemes such as Rapid Assessment programmes, Heart Failure and Respiratory Services and most recently, Enhanced Care at Home cheme.
